Greetings, sorry about your pain. Going by your description you have rheumatoid arthritis.  To confirm it you should do rheumatoid tests - RA FACTOR, C-REACTIVE PROTEIN, VITAMIN D3, VITAMIN B12, AND HEMOGLOBIN AND ALSO SERUM FERRITIN. these will help you identify if you have RA AND what to do next.
Next Steps
a diet with combination of protein and fat is most recommended. this will help fight joint swelling and exercise like walking and ball catching bouncing on floor will help initially.

Your condition seems like Rheumatoid Arthritis. Please consult a Rheumatologist and have your blood investigations done. Contrast bath, which is alternate heat (5 mins)  followed by cold (5 mins) helps alleviate pain. Once pain is gone please consult a Physiotherapist and learn Range of Motion exercises for all joints.

Please consult a rheumatologist. You have to get bloodtests done like anticcp, RA factor, ESR, etc and you require disease modifying drugs uf rheumatoid arthritis is positive.
