please stay calm. based on what you’ve described bleeding on the surface of the stool for 3 months and a swelling that appears only when straining it sounds like a manageable situation if handled correctly now. In Grade 2 the piles or cushions come out only when you strain like in photo but they go back inside on their own once you stop. This stage often responds very well to lifestyle changes and the right medication without needing surgery
Next Steps
make your stool soft and mushy and stop the bleeding, you need to reduce the pressure during bowel movement. Never sit on the toilet for more than 5 minutes. If it doesn't come, walk out and try later. Straining is what causes the bleeding.
Health Tips
Piles treatment is not one size fits all. we need to fix the root cause which could be your diet, hydration, or even how you sit. If we don't manage this now, Grade 2 can quickly turn into Grade 3, where the pain increases and surgery becomes more likely. schedule a private consultation so I can create a custom diet and recovery plan .

Looks like early internal piles (likely Grade 1–2) since bleeding is present and prolapse only on straining. You can take a stool softener like lactulose syrup (15–20 ml at night) or ispaghula husk (1–2 tsp with water daily), plus drink more water and avoid straining. If bleeding continues or increases, consult for proper examination and treatment—I can guide you further.

From your history: • Internal piles • Bleeding on stool surface • Visible swelling only on straining
Health Tips
• Diet is key: – High fiber (fruits, vegetables, oats) – 2–3 liters water daily • Toilet habits: – Do not sit >5–7 minutes – Go when urge comes (don’t delay) • Avoid triggers: – Spicy junk food – Constipation ✔️ Goal: soft, “toothpaste-like” stool (no straining) ⸻

As seen from the pictures that you have shared with the diagnosis that you provided. It seems as Grade 1 internal haemorrhoids, which are usually painless.

This stage requires only basic lifestyle modifications - eating healthy diets( avoid spicy, fried and highly processed food) -Stool softeners( Lactulose- 15 ml once a day, at night, after meals, can be mixed with milk /water- for 1 week) - stool Bulking agents(isapghula husk- 1-2 teaspoons, mix with 1 glass lukewarm water, drink immediately, and drink another glass of water after it, once a day at night- for 1 week) -high fibre diet (include salads, fruits, chia seeds in diet) -adequate hydration (2-3 L of total water daily) -Avoid straining while defecating
Health Tips
With stool bulking agents- ensure proper hydration
