Your BP is high which needs to be lowered to 130/80 .You can take telmisartan 40 mg twice daily or add a new antihypertensive from different class which ideally should be done under the supervision of your doctor.

Based on the information provided, here are my recommendations: Opinion: 1. The elevated blood pressure reading of 165/100 mmHg is considered high and needs to be addressed promptly, as uncontrolled high blood pressure can have serious health consequences. 2. Taking an additional blood pressure medication 12 hours after your regular dose of Telmisartan was an appropriate step to help bring your blood pressure back to a safer range. 3. However, it is not advisable to continue taking two doses of blood pressure medications regularly without consulting your doctor. Next Steps: 1. Monitor your blood pressure closely over the next few days. If it remains elevated (above 140/90 mmHg), contact your doctor promptly. 2. Do not continue taking two doses of blood pressure medications on your own. Inform your doctor about the recent spike in your blood pressure and the additional dose you took. 3. Your doctor may: a. Adjust the dosage or frequency of your current medication(s) b. Add or change the blood pressure medication(s) in your regimen c. Recommend lifestyle modifications, such as dietary changes, exercise, or stress management techniques 4. If your blood pressure readings return to a normal range (below 140/90 mmHg) with your regular daily dose of Telmisartan, you can continue taking it as prescribed by your doctor. 5. In cases of extreme heat or high stress, take necessary precautions to avoid exacerbating your blood pressure, such as staying hydrated, limiting physical activity, and practicing relaxation techniques. 6. Follow up with your doctor regularly for monitoring and adjustments to your blood pressure management plan as needed. It is crucial not to make changes to your blood pressure medication regimen without consulting your doctor, as improper dosing or abrupt discontinuation can have adverse effects. With proper medical guidance and lifestyle modifications, your blood pressure can be effectively managed.
