Groin, Scrotal & Anal Rash
Hello Doctor, I have been experiencing persistent itching around my buttocks, especially along the anal/butt crack, for the past 1–2 weeks. I have also noticed some red rash-like marks around my waist. Similar red marks have appeared on my scrotum, the area beneath the scrotum extending toward the anal region, and on my inner thighs. In addition, I have been noticing a significant unpleasant odor coming from the private area. The itching has become quite uncomfortable and is causing me significant discomfort. Could you please advise me on what this might be and recommend the appropriate medicine or treatment? I have also attached photos of both the front and back private areas for your assessment. I have slightly blurred the penis area in the front image for privacy purposes. Could you please review the images and let me know if you can see any visible rashes, marks, or other abnormalities? Thank you.

Based on your history and the attached images, there appears to be redness involving the groin, scrotal folds, and perianal region. While a definitive diagnosis cannot be made from photographs alone, the findings are most suggestive of a fungal infection (tinea cruris/intertrigo), other possibilities might be scabies, contact dermatitis, or a secondary bacterial infection, particularly in view of the unpleasant odor. I recommend an in-person examination by a dermatologist  for confirmation. Is the itching worse at night? Do you have similar itching or rashes elsewhere on your body, or do any family members have similar symptoms? Do you have diabetes or any other medical condition? Have you used any creams or medications on the rash already?
Next Steps
If a fungal infection is confirmed, treatment usually includes a topical antifungal cream (such as luliconazole, terbinafine, or clotrimazole) for 2–4 weeks. Extensive or persistent infections may require oral antifungal medication prescribed after examination. If you develop fever, pus, rapidly spreading redness, severe pain, swelling, or if the rash does not improve within 1–2 weeks of appropriate treatment, seek medical evaluation promptly.
Health Tips
Keep the affected area clean and thoroughly dry. Wear loose-fitting cotton underwear and change it daily. Avoid scratching and avoid using steroid-containing combination creams without medical advice, as they can worsen fungal infections.

This looks like Tinea Cruris, commonly known as jock itch — a fungal infection of the groin, inner thighs, and surrounding areas. The combination of itching, red rash spreading from the groin to the scrotal and anal region, and an unpleasant odor are all classic signs of a dermatophyte fungal infection. Warm, moist areas like the groin are prone to this, especially in warm weather or with heavy sweating.
Next Steps
Keep the area clean and dry, wear loose cotton underwear, and avoid sharing towels. An antifungal cream applied twice daily for 2-4 weeks is the standard treatment — consult a doctor for a proper prescription. Please consult me directly on Practo for a detailed evaluation.
