My Recommendation:
Surgical Oncologist: This is generally the preferred choice for metastatic cancer. They have specialized expertise in 'radical' clearances and managing the spread of the disease to other tissues.
ENT (Head & Neck Specialist): You can also consult an ENT surgeon, provided they specialize specifically in Head & Neck Oncology. A general ENT who mostly does ear or sinus surgeries may not be the best fit for a metastatic case.
Key Questions to ask your Surgeon:
How many thyroidectomies for cancer do you perform annually? (Look for high-volume surgeons).
Will a neck dissection be performed during the same surgery to remove metastatic lymph nodes?
How do you plan to protect the voice box nerve (Recurrent Laryngeal Nerve) during the procedure?

If it’s non cancerous lesion, surgical oncologist is not required.
