It depends upon your symptoms and whether you are planning to conceive. If you don't have severe pain or excessive bleeding during periods,  surgery is not immediately warranted.

Hello, I understand that you must have undergone an ultrasound scan, for your PCOD and had an accidental discovery of intramural  fibroid? Now you have some options 1) if you want to conceive then the treatment plan will be specifically different 2) if you do not wish to conceive then again the line of  treatment will again differ. 3) depending on the range of your symptoms, the treatment plan will be adjusted.
Next Steps
Consult with all of your past reports for better understanding.

If you are married and you are unable to conceive then go for laparoscopic removal of fibroid and along with that pcos drilling in the same setting.

The fibroid is small one ...you don't need surgery unless it is causing any symptoms.

You may not need any surgery at all. What are your symptoms? Why did you have a sonography done? Please consult a Gynaecologist with the answer to these questions.

Intramural fibroids are operated only if they are symptomatic like heavy bleeding not controlled on tablets or difficulty in conceiving. So in case you are asymptomatic no need to get it operated
