Hello. My report says:
• "Right Sided Pleural Effusion with underlying minimal consolidation, Effusion measures approx 800-900 ml. No pleural effusion seen on the left side."
• ADA (Pleural Fluid):- 56.8 U/L
• MTB-RIF Assay:- "MTB NOT DETECTED".
I started my treatment with the standard 'NTEP' medication on 20th July 2023. The doctor did not aspirate the entire pleural fluid out. They just took a syringe sample out for testing.
After 20-22 days of regular medication, I'm feeling a bit unwell. I feel slight heaviness with a mild vibration in my right chest when I inhale. Also, there's a slight growth in my coughing frequency. For a few days after medication, my coughing was negligible. I also feel nausea, fatigue, and some gastric problems such as acidity and vomiting.
I take medicine (4 tabs) around 9 AM everyday with 5 minute gaps between each pill. One day I forgot to take medicines around 9 and took it between 9.30-9.45 AM. Is there any problem due to that? Please suggest.

Get fresh CXR and Chest Ultrasound for the amount of fluid. If the fluid is still around 800-900 ml and you are feeling discomfort in chest , then pleural fluid needs to be drained out through syringes. At least drain around 500ml fluid .
ATT takes time to be completely effective to reduce pleural fluid amount.
