Structured adjustments to diet, exercise, and body weight are the most effective ways to reduce liver inflammation, fatty liver and prevent scarring.  I would suggest these 5 points for you to include into your day-to-day routine. 1. Aim for Gradual, Targeted Weight Loss 10% of your weight..... Around 8kgs weight loss will definitely help you reverse fatty liver. Keep weight loss steady at 0.5 to 1 kg per week, as rapid crash dieting actually worsens liver damage. You have  high BMI as well. You need to bring it back to normal range. 2. Cut Out Fructose, Refined Carbs, and Alcohol. 3. Adopt a Mediterranean-Style Diet Liver-Friendly Fats: Use extra virgin olive oil and eat daily handfuls of walnuts or almonds for protective omega-3 fats. Load Up on Greens: Eat plenty of broccoli, spinach, and kale, which contain natural antioxidants that stop fat buildup. 4. Build a Consistent, Mixed Exercise Routine considering your spasms, you can still do  moderate aerobic exercise weekly, like brisk walking, cycling, or swimming. Even these doesn't work, still Keep Moving Independent of weight loss, staying physically active burns up circulating sugars before they can turn into liver fat 5. Control Your Metabolic and Medical Health . focus on reducing belly fat and gaining muscle rather than general weight loss.

Your age of 38-year-old male, 5'8" tall, weighing 84 kg (BMI ~27.5, indicating overweight) with Grade 2 fatty liver focuses primarily on: 1. Gradual, sustained weight loss of 7–10% of total body weight 2. A balanced fiber-rich diet 3. Regular physical activity to reduce liver fat and inflammation. Aim to lose about 6 to 8 kg gradually (0.5 to 1 kg per week). Avoid rapid crash diets. Do 150 to 300 minutes of moderate aerobic exercise (like brisk walking or cycling) plus strength training weekly. Get 7–8 hours of quality sleep and practice stress management to support metabolic health. Emphasize whole grains (oats, brown rice), fresh vegetables, fruits, lean proteins (fish, chicken, legumes), and healthy fats (olive oil, nuts). Eliminate sugary drinks, sodas, packaged sweets, refined carbohydrates (white bread, maida), fried/oily foods, and high-fructose items. Strictly stop all alcohol consumption as it speeds up liver damage. There is no single pill to cure fatty liver. Doctors may prescribe treatments if you have high cholesterol, high blood pressure, or pre-diabetes/insulin resistance. Specific medical options or supplements like Vitamin E are evaluated on a case-by-case basis depending on blood tests and liver stiffness.u

Fatty liver grade 2 with a sedentary night-shift lifestyle is very common — and very reversible. There's no specific approved medicine for fatty liver; weight loss of even 5–10% can reduce liver fat significantly. Night shifts disrupt metabolism and worsen the condition.
Next Steps
Aim to lose 5–7 kg through diet changes — cut sugar, refined carbs, fried food, and alcohol. Walk 30 minutes daily even at home. For spasms preventing exercise, start with stretching or yoga. Get LFT, lipid profile, and HbA1c done. Consult me on Practo for a structured diet and management plan.
