A pea-sized, long-standing lesion that feels firm and contains trapped material is likely not a simple acne pimple. It could be a closed comedone, a small cyst, or a keratin-filled lesion where the contents are not coming out on their own. Since it has been present for 8 months and did not respond to benzoyl peroxide, it needs proper evaluation.
Next Steps
Do not try to squeeze or puncture it, as this can lead to infection, scarring, or pigmentation. Topical treatments alone may not be enough in such cases. A dermatologist may need to assess it and perform a simple in-clinic procedure such as extraction or minor drainage under sterile conditions. In some cases, additional topical or oral medications may be advised depending on the diagnosis.
Health Tips
Keep the area clean and avoid applying heavy or comedogenic products over it. Continue using a gentle cleanser and sunscreen, and avoid repeated touching or manipulation of the lesion. If it becomes painful, enlarges, or shows any changes, consult a dermatologist promptly for proper treatment and to prevent any permanent marks.

Acne or pimples... Due to hormonal changes..Oily skin causes it...Common in adolescent age...May occur in adults also.. Food like Oily foods, ice cream, chocolate and sweets increase it.. Treatment depends on the grade of pimples or acne..So, please send photos by direct online consultation as it's a must to see which grade of pimples or acne for accurate diagnosis and treatment.
This sounds like a closed comedone / cystic acne (deep-seated lesion) where pus is trapped inside and not coming out. That’s why it feels tight and has been present for a long time. It is not advisable to force the pus out.
Helpful Tips / Word of Caution:
• Do NOT squeeze or prick it → can cause scarring
• These lesions take time to resolve (few weeks)
• Maintain a simple skincare routine (avoid heavy creams/makeup)
