Get serum electrolytes, serum calcium,  vit d, b12, checked and share results.   Instead of taking Newtel am,( telmisartan 40 and amlodipine 5), take tab telmisartan 20mg, and amlodipine 5 mg in morning. Telmisartan does reduce uric acid as well. Avoid beer, spinach, tomatoes, peas, beans. Repeat uric acid after 4 weeks. For muscle cramp,  take tab evion 400mg one morning and one evening for 4 weeks.

Your BP is on the lower side, which may be causing dizziness and sleepiness. Dose adjustment may be needed, and uric acid can rise with some BP medicines. Please consult me so I can review your medicines and guide you safely.

You are taking Newtel AM twice daily (Telmisartan + Amlodipine combination). Current situation: • BP = 110/65 mmHg • Dizziness • Sleepiness • Muscle cramps • Uric acid = 7.9 mg/dL Interpretation: 1. BP 110/65 with symptoms → likely over-control of blood pressure. Even though 110/65 is not dangerously low, if you are symptomatic, the dose may be slightly high for you. 2. Muscle cramps → may suggest: • Electrolyte imbalance (especially potassium or magnesium) • Mild dehydration • BP slightly low 3. Uric acid 7.9 → Elevated. Telmisartan usually does not raise uric acid significantly, but: • Dehydration can increase uric acid. • BP medicines may indirectly affect kidney handling of uric acid. So yes — dose reassessment is reasonable, especially since you are symptomatic. This is not an emergency, but it should not be ignored.
Next Steps
✔ Measure BP twice daily for 5 days (morning & evening). ✔ Check: • Serum electrolytes (Na, K) • CreatinineUric acid repeat ✔ Increase hydration unless restricted. If average BP remains: •
Health Tips
✔ Stand up slowly from sitting/lying ✔ Drink 2.5–3 liters water daily (if no kidney issue) ✔ Reduce red meat, organ meat, alcohol ✔ Avoid crash dieting ✔ Monitor for ankle swelling (amlodipine side effect) Since you are experiencing symptoms and have rising uric acid while on antihypertensive therapy, medication optimization is important to prevent long-term complications. I strongly recommend booking an online consultation so we can review your BP logs, lab values, and adjust your treatment safely rather than guessing the dose.

Dear Patient, You are taking Newtel AM twice daily for high blood pressure. Your current blood pressure reading is 110/65 mmHg, which is on the lower side. Symptoms such as dizziness, sleepiness, and muscle cramps may occur when blood pressure becomes lower than your body requires or due to electrolyte imbalance. It is possible that your current dose is higher than needed, and dose adjustment may be required. You should not change the medication on your own without medical advice. Newtel AM contains telmisartan and amlodipine. Some blood pressure medicines (especially those containing diuretics, if taken earlier) can increase uric acid levels. A uric acid level of 7.9 mg/dL is mildly elevated and should be monitored, along with diet modification and hydration. What you should do: 1. Check blood pressure regularly (morning and evening readings for a few days). 2. Get blood tests done — serum electrolytes, calcium, vitamin D, and vitamin B12. 3. Maintain adequate hydration. 4. Avoid sudden standing from sitting or lying position to prevent dizziness. 5. Consult your doctor regarding possible dose reduction or single-dose therapy.
Next Steps
If dizziness is severe, fainting occurs, or BP falls below 100 systolic, seek medical attention promptly.
