U r anemic  is the most important finding. Others are for academic interest.   Caplet haem up one at night after dinner for two months . Colour of stool will change.  Don't worry. Folic acid5 5 mg two times a day after food for 2 months. Before starting therapy  get yourself  deworm. JUST ONE TAB ZENTAL400 YO BE CHEW FINLAY WITH A GLASS OF WATER BEFORE BREAKFAST. FROM SECOND  DAY START THERAPY. PLENTY OF WATER N LIQUID LEMONADE ELECTORAL POWDER BUTTERMILK TO BE CONSUMED.

You have iron deficiency anemia. Take more of fresh green vegetables,  beetroot,  pumpkin,  fresh seasonal fruits, use jaggery as sweetener. Take syp maltviron 5ml every morning and evening for 6 weeks. Tab vitamin c 500mg one morning and one evening for 20 days.
