Itching around the anal region could be due to a number of reasons including worms. To be certain if it's due to worms, get a stool analysis for ova and cysts and treat accordingly.
Thanks for sharing your details. Since you're experiencing persistent itching (especially at night) and Albendazole 400 mg hasn’t worked this time, here’s a clearer medical perspective:
Persistent anal itching, especially at night, can be caused by:
1. Pinworm (Enterobius vermicularis) – Most common cause.
2. Other parasitic infections – Like strongyloides, giardia, etc.
3. Fungal infections – Especially in warm, moist conditions.
4. Hemorrhoids or anal fissures – Less likely if no pain or blood.
5. Skin irritation – From soaps, hygiene products, or diet.
6. Poor response to Albendazole – Possibly due to reinfection or resistance.
Next Steps
Try Mebendazole or Ivermectin instead of Albendazole:
Option A: Mebendazole (Vermox)
100 mg twice daily for 3 days.
Repeat after 2 weeks to prevent reinfection.
Option B: Ivermectin
Single dose: 200 mcg/kg body weight (approx 12 mg for 60–70 kg).
Repeat after 2 weeks.
General Hygiene Advice
Wash hands thoroughly after using the toilet and before eating.
Keep fingernails trimmed to avoid egg spread through scratching.
Wash underclothes and bed linen in hot water.
Bathe in the morning to reduce egg transmission.
Stool Test (if unresolved)
If symptoms persist after trying a different dewormer, request a stool ova & parasite test (O&P) to rule out other infections or causes.

Perianal itching is not only due to worm. There can be different cause. Fungal or dry skin can cause the symptoms. Use an antifungal gel and steroid cream for 1 to 2 weeks
Dear Practo User.
Thanks for sending across your query.
If Albendazole 400 mg isn't relieving your symptoms, it may be resistant worms or another cause like piles or infection. Consider repeating the dose after 2 weeks or using Mebendazole. A stool test and doctor visit are recommended.
