As u mentioned.....becaz of regular smoking...layer of tobacco starts getting settled on teeth n around it ....gradually it gets hard n may get absorbed on teeth surface at times
Tooth paste may help only after treatment of cleaning n polishing

As you said dark spot-it could be due to caries or stains from smoking-called stains.Need further information,intra oral photographs and examination.

As from your information stains due to tobacco could be removed by scaling and whiteness could be achieved by bleaching.Whereas the other one caries to be excavated and filled.
Stains from smoking can be removed only with a professional cleaning. Visit your dentist for a Scaling. Regular brushing would not help you much with stubborn stains and hard deposits (calculus).

Dark spots will mostly be stains which have occurred due to smoking.no paste can remove that. And plz don't try to apply pressure while brushing to remove the stains as that will remove the enamel instead.and later cause sensitivity problem.
