Critically low hemoglobin - 5.8
My wife is suffering from low hemoglobin levels since 1 year. Levels fell down 5 months after marriage due to continuous bleeding for a month, she has thyroid, pcos. With all medicines, food & juices her fatigue reduced, thyroid in control and hemoglobin got increased to 7 in December 2024. Now since 1 month again she is having continuous bleeding & dizziness, fatigue, low BP, high pulse, hemoglobin at 5.8.. Family doctor asked to take Iron Injections.. at least 5 .. Pls suggest if thats ok ? Will there be any side effects? And what would be the cost ?

To the Patient:** Your wife's critically low hemoglobin (5.8 g/dL) with persistent bleeding requires urgent attention. Here's my professional assessment: Critical Concerns:** 1. **Severe Anemia Risks:**    - Symptoms (dizziness, fatigue, tachycardia) indicate her body is struggling to oxygenate    - This level can strain heart function and requires prompt treatment 2. Underlying Causes:**    - PCOS + thyroid disorder likely contributing to menstrual bleeding irregularities    - Potential iron/B12/folate deficiency from chronic blood loss Treatment Plan:** 1. Immediate Action:**    - Iron injections (e.g., Iron Sucrose) are appropriate and necessary at this stage    - Typically 5-10 doses (cost ≈ ₹300-500 per injection)    - Minor side effects may include nausea or metallic taste 2. Concurrent Management:**    - Must control abnormal uterine bleeding (may need hormonal therapy)    - Continue thyroid medication optimization    - Start oral iron supplements post-injection course 3. Monitoring:**    - Weekly CBC checks until hemoglobin stabilizes    - Investigate for other bleeding causes if persists Urgent Consultation Needed:** This is a medical emergency requiring: - Complete hematological workup - Gynecological evaluation for bleeding source - Personalized transfusion/injection plan For Immediate Guidance:** Contact me directly to coordinate her care. **My WhatsApp:** Eight Zero Zero Nine Eight Two One Three Three Two

You can take iron injections. Since you have taken them before, you can take them again safely. After 3 to 5 injections, continue with iron tablets Take vitamin c 500mg one morning and one evening for a month on empty stomach,  as that would be beneficial. Cost you can ask from a pharmacist.
Next Steps
Adv- gynaec consultation for excess bleeding.
Health Tips
Take pomegranates, green leafy veggies, beetroot,  jaggery etc which provide iron naturally

Yes , please take iron injections soon There is a iron deficit of around 1500 mg Almost 3 injections , each of 500 mg per dose will be sufficient Each may cost around 2000 rupees Consult  gynecologist for evaluation of vaginal bleeding as that needs to be treated , otherwise taking iron injections will be in vain

Hemoglobin level of 5 is significantly low ,may need blood transfusion to improve her hemoglobin level, but the actual cause of bleeding needs to be evaluated and appropriately managed by consulting a gyenocologist.

Hi firstly take iron injections alternate day and once she’s stabilised go for a sonography of pelvis to know the exact cause of bleeding . One month bleeding is considered as abnormal uterine bleeding and should be evaluated thouroughly
