Continous fever and empirically on ATT
Had cough 25 days ago, after that low grade fever started from 15 days ago.complete body aches from lower back to head and by the time fever reaches 100.5 pcm was taken 2-3 times daily sometimes four times. suffers  from hypertension and type-2 diabetes ,had underwent CABG in 2024.Also had a history of peritoneal TB in 2018.currently pleural effusion(right>left), pericardial effusion and mild ascites seen in my cect abdomen whereas rest all reports are normal but still fever persisting. PCT values for 19 sept is 0.23.ESR for 17 sept is 23.Doc started ATT empirically from 25 sept. Now fever comes once in day with max upto 100F.Body aches has nearly subdided with minimal persisting some times. ESR on 27 sept is 46 and on 1oct is 61.is my ATT working.

Prior history of TB, makes starting ATT look like a correct decision. The course should have been started ideally after testing the bacterial resistance to drugs. But that now the symptoms have subsided, mostly it’s drug sensitive TB. The course should be taken for 6-9months depending on the progress, intensive phase (IP) for 2 months and continuous phase (CP) for 4 months. Initially acidity and nausea are common side effects, in such instances syrup suclarfate 1 tbs 30mkn before meals can be taken. Always look out for side effects while taking ATT- - isoniazid causes pheripheral neuropathy, so always take tab pyridoxine(vit B6) 20mg once daily, already a known case of diabetes increases the risk -pyrazinamide increases the uric acid levels in the body and causes severe joint pain and lever toxicity as well - ethambutol can causes decreased vision and scotomas - rifampicin causes liver toxicity, causes red urine (comparatively less harmful) Have rich protein diet(especially 2 eggs)daily and never miss you daily doses of ATT

Yes ATT is working. Superadded infections (mostly viral/ dengue) may cause fluid collection in many areas at the same time. Since the symptoms are decreasing in frequency and intensity, there’s nothing to worry. Keep your diet balanced and consider adequate fluid intake.

Yes your att is working ..its just super added infection from the pleural effusion. Pleural effusion fluid tests to be done. Fever on and off can be a part of the att treatment too. Kindly get a cardiac consultation done too in case of breathlessness
