It is common for these conditions to be chronic, meaning they can last for months or years and may flare up even after a month of medication. Medications like Proton Pump Inhibitors (PPIs): Such as omeprazole or esomeprazole, to block acid production and allow the lining to heal. Antibiotics: If a breath or stool test confirms an H. pylori bacterial infection. H2 Blockers: Such as famotidine, to reduce acid released into the digestive tract.
Next Steps
Dietary Adjustments: Avoid Triggers: Spicy, acidic (citrus/tomato), fried, or fatty foods. Small, Frequent Meals: To avoid putting too much pressure on the stomach Stop eating

Hello, It is understandable to be frustrated when symptoms like bad breath and burning sensations persist despite medical treatment. Your endoscopy results indicate significant inflammation in the lower part of your stomach (antral gastritis), the beginning of the small intestine (duodenitis), and damage to the food pipe (esophageal erosion). 1. The "Lax Incompetent" LES and Bad Breath The endoscopy mention of a "lax incompetent" cardiac sphincter (Lower Esophageal Sphincter) is very important. Mechanism: This valve normally stays closed to keep stomach acid and gases down. When it is "lax" or weak, stomach contents and acidic vapors can travel back up (reflux). Bad Breath (Halitosis): In your case, the severe bad breath is likely not an oral hygiene issue, but rather caused by these stomach gases and fermented material escaping through the weak sphincter. 2. Why Symptoms Return After Medication One month of medication is often just the beginning of healing for "erosions". If the underlying cause-such as H. pylori bacteria, high stress, or specific dietary triggers-is not fully managed, the inflammation will recur as soon as the acid-blocking medication wears off. Recommended Diagnostic & Treatment Steps: H. pylori Testing: If not already done during your endoscopy via a biopsy, you should have a Urea Breath Test or Stool Antigen Test. This bacteria is a leading cause of antral gastritis and must be treated with a specific antibiotic "quadruple therapy". Prokinetic Medications: Since your sphincter is lax, a doctor may add a "prokinetic" medication. This helps the stomach empty faster and strengthens the sphincter to prevent reflux. Follow-up Endoscopy: Given the esophageal erosions, a repeat endoscopy in 2–3 months may be needed to ensure the tissue is healing and not developing further complications. Management Advice: Dietary Discipline: Avoid the "Big Three" triggers: Caffeine (coffee/tea), spicy foods, and high-fat/fried meals. These further relax the sphincter and irritate erosions. Mechanical Prevention: Do not lie down for at least 3 hours after eating. Sleep with your head slightly elevated (using a wedge pillow) to let gravity keep stomach contents down. Smaller, Frequent Meals: Large meals put pressure on a lax sphincter. Eating smaller portions can significantly reduce the "bad breath" and burning sensations. Summary: Your symptoms are directly linked to the physical weakness of the stomach valve and ongoing inflammation. Success will require a combination of confirming an H. pylori diagnosis, using medications that aid stomach movement, and strict lifestyle changes to protect the esophagus. Best regards.

Your endoscopy findings suggest chronic acid-related disease — mainly gastritis, duodenitis, and reflux (due to lax/incompetent LES). This explains both your burning sensation and bad breath. Bad breath in your case is likely from acid reflux + inflammation + possible bacterial overgrowth, not just oral causes. Since symptoms are persisting after 1 month of treatment, it means either: 1)treatment duration was insufficient, or 2)H. pylori infection may not have been addressed, or 3)lifestyle triggers are continuing
Next Steps
-Start/continue a proper course of PPI (like Pantoprazole/Esomeprazole) for at least 6–8 weeks (take consultation for dosage and prescription) -Get tested for H. pylori (if not already done) -If positive → take eradication therapy (antibiotics + PPI) -Add a prokinetic (like levosulpiride/domperidone) if reflux symptoms are significant -Consider dental check if bad breath persists
Health Tips
•Avoid trigger foods: spicy, oily, caffeine, carbonated drinks •Do not lie down immediately after meals (keep 2–3 hour gap) •Eat small, frequent meals instead of large meals •Elevate head end of bed slightly •Avoid smoking/alcohol if applicable •Stress and irregular meals can worsen symptoms significantly This condition is treatable but needs consistent therapy and lifestyle correction. With the right approach, you should see clear improvement.
