You seem to be having perianal fistula which is intermittently causing an abscess to form resulting in the pain that you are experiencing. I would suggest you to. Start a Course of antibiotics
Tablet augmenting 625mg twice a Day
Tablet Calpol 650mg if you have pain else it can be skipped
Capsule pan 40 mg once a day before breakfast
Sirz bath ( take a tub filled with lukewarm water and add half a teaspoon of salt into it and sit in the tub so that the affected area gets dampened by the salt water, do this twice a day - once in the morning and once in night)
If you are having hard stools which increase your pain then please consume a diet rich in fiber such is Fruits and vegetables and avoid coffee and non vegetarian food for 2 weeks)

Visit Surgeon if no improvement

Tab Augmentin 625mg thrice daily for 7days
