I feel like I am always anxious. Even though I don't feel much physical symptoms but in my social behavior it's very evident. I only feel comfortable in my own vicinity, the moment I go out, my behavior changes and I start to act differently. I am not very good in social gatherings even with my close friends. I get very nervous where I have to take any decisions or in any pressured situation.

Hello!
Anxiety could be emerging due to multiple reasons. It could be because of past events or perhaps you need to develop skills so as to not act differently in social gatherings. Both the factors and any other factor resulting in anxiety can be dealt with by visiting a psychotherapist/cognitive hypnotherapist/NLP practitioner. You can even try doing relaxation exercises and breathing exercises. They might help you feel calmer.

You seem to be suffering from anxiety disorder, likely social anxiety. Please contact a psychiatrist in person for a detailed evaluation which will include physical examination to rule out any physical cause for anxiety as well as a psychological evaluation. Once the cause of anxiety is established, we will be able to focus on the treatment part which may consist of medication and psychological therapy.
