Anal and groin area itching
Hello Doctor, I have been experiencing itching for quite a long time in the groin area (inner thighs near the penis) and around the anal region. The itching becomes especially intense when I go to bed, whether during the day or at night. I have already taken antifungal medication (AF-150) and used clotrimazole (Candid cream), and Luliconazole but there has been no improvement. There are no clear red rashes or ring-like marks, but the skin has become irritated and slightly darker due to scratching. Could you please guide me on what this condition might be and what treatment I should follow?

This is most likely NOT a fungal infection (since proper antifungal treatment has already failed). • Most likely diagnosis: • Lichen simplex chronicus (itch–scratch cycle) • Pruritus ani (chronic anal itching) • Possible irritant/contact dermatitis (sweat, friction, soaps) Why this fits your case: • No ring-shaped rash → fungal less likely • Long duration + no response to antifungals • Skin darkening → due to chronic scratching • Worse at night → classic itch cycle / pruritus ani
Next Steps
• Wear loose cotton underwear • Change underwear twice daily if sweating • Avoid tight clothes / gym wear for long hours • No harsh soaps, dettol, or antiseptics • Do NOT scratch → worsens pigmentation + itch Kindly consult with me via Practo for detailed evaluation and treatment

Hey there.... This appears a catch 22 situation. Are there any stressors in your life and /or a dry skin issue or co-morbidities like hypothyroidism or DM? Please send clear pics of the same. A detailed history of your problem needs to be delved into before a clear management pathway is structured.
