Plaque is thin film cant be seen from naked eyes! So be assure its more than plaque! Scaling and polishing will be answer to your bad breath, senstivity and bleeding! There might be other reasons for senstivity other than calculus, whuch has to be addressed if senstivity doesnt resolve! Regarding the hole , consult a dentist

Hi mam..     plaque development is due to poor oral hygiene....   plaque development  may leads to gingivitis (inflammation and bleeding from gums)    If you left the plaque development it may  cause gingivitis,periodontitis, bone loss leads to tooth mobility,tooth loss....
