Thyroid function tests should be done at the earliest. Hope there was no fever in the recent past A complete blood count and peripheral smear needs to be done as well. Consult. If frequent might have to do Holter monitoring or Webcardio after all baseline blood investigations.
Next Steps

Your Haemoglobin level is 18.4 gm% . I am sure if you get your CBC done you will also be found to have polycythemia. It can cause miniature clots in the vessels. You have got ECG, 2D ECHO, and TMT done. At least two of these tests are done by the Cardiologist. Please consult your Cardiologist on this line, could it be due to Polycythemia ?
