Dear Doctor,
My wife had a front tooth removed six years ago due to an infection, with a screw and cap implanted. She has now been advised to remove the structure and get a new implant, with a temporary cap that must be removed nightly.
Since she is currently outside India, I would appreciate your opinion on whether this is the best approach or if alternative options exist.
I have attached the radiograph image for your review. Looking forward to your guidance.
Thanks,
Vaibhav

this seems like a post and core with a cap(crown) on top. we first need to assess if the cap merits removal. when that is confirmed only then can the next process be discussed. New implant may or may not be necessary. please understand an implant is root analog and is a significantly different process that has happened in this case
Next Steps
radiographic and physical examination by a prosthodontist
Health Tips
try not to bite too hard on that tooth to avoid any trauma
I can offer general advice here, a proper diagnosis often requires a more thorough evaluation.
The recommendation to remove the existing implant and place a new one with a removable temporary cap is one possible approach (usually up to 3mos till implant is ready to receive permanent cap), often indicated by issues like infection or implant failure.
Alternative options might include
addressing the existing restoration (if feasible),
different implant techniques,
a fixed temporary restoration (depending on the situation),
dental bridge (involving adjacent teeth),
or a removable partial denture(to be removed every night)
Health Tips
Best advice can be given after checking the patient in person .
Consider having a detailed discussion with the dentist.
or could visit for a second opinion
Hello!
According to x-ray there is infection, RE-RCT can be initiated only if the tooth structure is sound and healthy, if not extraction followed by implants is the other option
