There is 2 possibilities 1) it is phimosis with balanoposthitis because of extra skin on penis with infection 2) insects bite
Next Steps
Keep your kid in warm water tub for 15 minutes 2 times to sooth the swelling, apply Neosporin ointment twice daily and massage foreskin of penis Photograph will clear the diagnosis further

Swelling could be due to infection of the penile head or the prepuce. Is the baby passing urine adequately, is there any burning micturition, were you able to retract the skin before these are some questions to be answered.
Next Steps
Child requires atleast a physical examination to rule out ant surgical cause and he may require oral antibiotics too.
Health Tips
Consult a pediatrician

Please do not apply steroid cream as advised by other doctor. It will increase infection in long run. Consult paediatrician properly
