Tapering of medicines needs to be done systematically under the guidance of the prescribing Psychiatrist. If the symptoms recur despite gradual reduction as advised, then the rate of reduction needs to be made much more slower after stabilization of current symptoms. There are augmenting Neuromodulation interventions like TDCS and TMS available which can also be considered when medications dependence is present.
Next Steps
Discuss with the Psychiatrist about trial of reduction

Specify withdrawal symptoms There is no rationale for continuing clonazepam for such a long time Equivalent dose of chlordiazepoxide can be taken for a short while after stopping clonazepam (12.5mg equivalent for 0.25 mg clonazepam) and then stop Continue desvenlafaxine depends on other factors such as episode/family history/stressors etc. A thorough evaluation is definitely needed

Hi Sir. I would recommend you not to reduce the dosage on your own as the tapering of these medicines requires expertise. Consult a psychiatrist before taking any such step. If you should stop the medication or not, all depends on things like what history you have, how long standing the issues have been and many other factors. Only after that we can decide if we should reduce the dosage or by when we should do so.
Next Steps
Please consult a psychiatrist before changing any thing related to your medicines.
