Sharing an X-ray for better clarification. My jaw started hurting and on X-ray, it turns out it's the impacted tooth on my right lower jaw. I am aware that I have to get the lower wisdom teeth removed, but is it necessary to get the upper two removed as well?
They have never caused me any problem but one of the surgeons suggested getting all 4 of them removed at once, which might be very painful for me to recover from.

hello maam,the pain is because of third molar pushing your anterior teeth.Yes its adviceable to get all your third molars out.You can get one side done first and later on the other sie for your comfort zone.

Your chief complaint is pain the lower right jaw because of your impacted third molar.
That should be addressed first.
But as your oral surgeon said, upper ones need to be removed too..that is because our teeth are equally placed in the jaws..upper and lower. There is always a tooth in the lower jaw to counter another one in the upper. When the lower third molar is removed, the upper third molar will start drifting down into your mouth which is termed as supra-eruption.
On the radiograph your upper third molar isn't straight, it's positioned to the right side into your cheek..and when that starts to come, it'll start hitting your cheek, hence it would be good to take it out in one appointment.
And about your concern of getting all 4 removed..speak to your surgeon and do one side at a time.
Upper and lower right side first as it is your complaint..
And later left side.
It is always advisable for extraction of all 4 3rd molars as because 3rd molars a vestigeal teeth and of no use .
But as because you have your complaint only for right lower 3rd molar so elimination of patient complaint should be primary approch.
