No, surgery is not the only option. It is one of the options for wisdom tooth removal only when it is covered with bone or it is impacted. Otherwise if the decay is superficial,  it can be filled with restorative material. If the decay is deep , then we can go for RCTs. And if nothing can actually save the tooth or its causing discomfort , then it is advisable to extract or remove the wisdom tooth.
Next Steps
Ask your dentist , take appointment , sit and understand properly.
Health Tips
The nerve involvement will be the only risk factor in major cases. otherwise it's a normal routine procedure.

Not a big procedure! Depends upon your tooth position, the treatment varies! Attach your pics or consult online for more details!

RCT can be done if  there is enough tooth structure and that wisdom tooth is involved in function ( for chewing), but would be little costier. If the decay is so deep and causing infection always, it is better to extract. If the tooth is fully visible, then surgery is not needed, normal extraction procedure will do. If the tooth is completely damaged, then the difficulty level will be high. Nothing to worry about, if you follow the instructions after extraction. Hope this helps you
