Thank you for sharing your concern. Based on what you've described—fasting blood sugar between 220-250 mg/dL for the past 7 days, persistent weakness, and no improvement in body weight over the last 2 months (currently at 50 kg)—it’s important to address a few possibilities: 1. Uncontrolled Diabetes: Your fasting sugar levels are quite high, which indicates poor blood sugar control. This can lead to weight loss, fatigue, and increased risk of complications. 2. Possible Insulin Deficiency or Resistance: If you're on medication or insulin and still experiencing these levels, the dose or regimen may need adjustment. 3. Nutritional Deficiency or Malabsorption: Persistent high sugar can lead to nutrient loss through urine. This might be contributing to weight loss and body weakness. 4. Other Conditions: Sometimes, underlying infections, thyroid disorders, or digestive issues can also cause weight loss and fatigue.
Next Steps
Consult your doctor or diabetologist immediately to review your medication, insulin, and possibly get further tests (HbA1c, thyroid profile, CBC, liver/kidney function). Get a diet plan tailored for diabetes and weight gain — rich in good quality protein, complex carbs, and healthy fats. Stay hydrated, and avoid skipping meals. Regular monitoring of blood sugar (fasting, postprandial, and bedtime) is crucial right now.

Unexplained weight loss in uncontrolled diabetes often indicates insulin deficiency. If your blood sugars are high and you’re losing weight, it suggests that your current treatment may not be enough. It’s important to reassess your medications, how long you’ve had diabetes, and your overall condition. In my opinion, if you’re experiencing both weight loss and poor sugar control, you may be a candidate for starting insulin.
Next Steps
Please consult your doctor for a detailed evaluation.
