Yes. Dark stools are a concern. You should consult a gastroenterologist and plan for an endoscopy. Vitamin D and B12 correction is also needed.

Your symptoms most likely suggest acid reflux (GERD), which is commonly seen in people who are overweight. Since your ECG and ECHO are normal, a heart problem is unlikely. Dark stools are a concern only if they are truly black, sticky, and foul-smelling; many times stools appear dark brown and are mistaken for black. At this stage, an endoscopy is not immediately required unless there are warning signs or symptoms do not improve with treatment.
Next Steps
• Start an acid-reducing medicine (PPI) once daily before breakfast for 4 weeks • Get a stool occult blood test to rule out hidden bleeding • Take Vitamin D3 60,000 IU once weekly for 8–12 weeks • Try gradual weight reduction, avoid late-night meals, spicy and oily foods • Sleep with the head end slightly elevated
Health Tips
Acid reflux is very common and treatable. Most patients improve with medicines and lifestyle changes. Endoscopy is needed only if symptoms persist or warning signs appear.

• Your symptoms are most likely acid reflux (GERD) related to overweight and severe Vitamin D deficiency. • Vitamin D 5.5 is very low → needs treatment. • Vitamin B12 (240) is borderline low → supplementation advised.
Next Steps
Endoscopy? • NOT needed now if: • No true black tarry stools (melena) • No vomiting blood • No difficulty swallowing • No unexplained weight loss or anemia • DO endoscopy if: • Stool is jet black, sticky, foul-smelling • Pain persists >4–6 weeks despite treatment • New alarm symptoms appear
Health Tips
• Start PPI (e.g. pantoprazole) once daily before breakfast • Vitamin D3 loading dose (as per doctor) • Vitamin B12 supplementation • Weight reduction, avoid late meals, spicy/oily food, tea/coffee at night If symptoms don’t improve in 4 weeks → endoscopy.

You have borderline low vit b12 and much lower vit d3. Take tab vit d3 60,000units one per week for 4 weeks. Tab neurobion forte one daily for 4 weeks. Take small frequent meals, avoid late evening meals,  avoid fatty, spicy food, Tab pantaprazole 40mg one daily before breakfast for 2  weeks. Tab domperidone 10mg one every 8hours for 7 days. Syp digeplex 5ml morning and evening for 2 weeks. If heartburn is frequent or severe or long standing, go for upper G.I. endoscopy. Also do test for H.pylori and share results.

Black colour stool is a sign of upper GI bleed. If you are facing heart burn and black stool then you must definitely go for endoscopy. Some time ingestion of iron tablets also causes changes in stool colour. Better you go for endoscopy for better management.

Because you have chest burning + possible black stool, consulting a gastroenterologist is strongly recommended. They may: • First do stool occult blood + CBC • Then decide on upper GI endoscopy

Start anti-reflux treatment (as advised by a doctor): PPI (like pantoprazole / omeprazole) Lifestyle changes- Avoid tea, coffee, spicy, oily food. Weight reduction is very important. Correct deficiencies- Vitamin D supplementation (high dose needed.) Vitamin B12 supplementation. Stool occult blood test (simple, non-invasive). First try medical treatment for GERD. Endoscopy only if symptoms persists.

From my experience, this sounds more like acid reflux than a serious heart problem, especially with low Vitamin D and being overweight. Many patients feel chest burning and breathing discomfort due to acidity.
Health Tips
Before doing more tests or taking medicines on your own, it’s best to see a doctor for a proper examination. Most symptoms improve with the right treatment, lifestyle changes, and vitamin correction. Further tests should be done only if clinically advised.

Vitamin D is definitely low however the heartburn seems related to something else like gastritis? B12 is on the lower side. Have you been having gastritis for a long time? If you really feel the color of your stool is darker then yes, an endoscopy would be helpful for evaluation

Based on your symptoms of heartburn (especially if it's occurring while breathing, which could suggest something like acid reflux or another GI issue) and possible black stool (which can indicate upper gastrointestinal bleeding, like from an ulcer or irritation), along with your low vitamin D (severe deficiency at 5.5 ng/mL) and borderline-low B12 (240 pg/mL), an upper endoscopy is often recommended in guidelines to evaluate the esophagus, stomach, and duodenum for potential causes.Black or tarry stools are considered an "alarm symptom" that warrants prompt investigation, as they might signal bleeding higher in the digestive tract—dark brown stool is usually less concerning, but if it's truly black and sticky, that's more urgent. Persistent heartburn also supports this, as it could point to GERD or related complications that need direct visualization. Your normal cardiac tests (ECG, echo) are good news, ruling out heart problems, and the negative tumor markers (CA125, CA15-3, CEA) reduce cancer concerns, but endoscopy can still help diagnose non-cancer issues like inflammation or absorption problems contributing to your vitamin deficiencies. At age 27 and overweight (BMI around 35, which is obese class I), factors like diet, lifestyle, or even medications could be playing a role—losing weight might help with heartburn long-term. In the meantime, supplement your vitamins as prescribed (e.g., high-dose D3 and B12 if advised), stay hydrated, avoid spicy/fatty foods, and elevate your head while sleeping to ease reflux.Definitely consult a gastroenterologist in Bangalore soon—they can decide if endoscopy is the next step based on a full exam. Places like Manipal Hospital or Apollo have good GI departments. Don't delay if the stool turns clearly black or you notice blood, vomiting, or worsening pain.
