Huge pain passing stool with no relief from Annovate suggests severe constipation or anal fissure. Need evaluation. Try: stool softeners (lactulose), increase fiber/water, warm sitz baths. Use Annovate correctly (inside rectum). If bleeding, severe pain, or no improvement in 2-3 days, see doctor for examination. May need different treatment.
As per your history problem is low fibre and sendentary lifestyle. Add more fibre and 3-4 litre of water in daily life. If fibre is not sufficient with diet ,take isabgol . If problem persists consult.
Include fibre in ur diet, increase water intake atleast 4 litres per day, reduce refined foods, regular excercise 30 min to 1 hr daily, avoid sitting for long hours
Next Steps
get up after sitting for 45 min and walk for 5 min
consult me for further treatment

Hi
Can understand how difficult it is for you
How long do you have this problem?
It can either be chronic constipation due to your fibre-poor diet or it can be fissure in ano
From your history I assume that you have had a very sedentary lifestyle
Lifestyle modifications would help you a lot to overcome this problem
Like early morning jogging, consumption of fibre rich food ( spinach and vegetables), drinking more water and avoiding multipurpose flour and items made from them. Drink a glass of milk before you sleep which contains lactulose and helps in easy defecation
Consult with me for further information and treatment
