I am on, telmisartan 40 MG since a long time, and my BP is well controlled
But the problem is that there are some days when it will stay elevated in the range of 140/80 ( I measure it at home, so might not be very accurate)
Also, if I check during periods of stress, then it can be as high as 150/90
Just wanted to know if this is considered normal ? The true BP measurement on waking up without any stress or effect of food will be normal.

From the question you Posted, I consider 1st change or treatment that you need would be stress management and lifestyle management ( regular exercise, sleep and salt restriction).
2nd thing is I'm concerned about whether you measured BP at home accurately or not. Coz loose or tight cuff, both may show increased BP. Please check your BP at a hospital or clinic atleast once.
3rd get your lipid profile, ECG ,2D echo and renal function tests to look for other causes. High cholesterol, long standing BP can change heart size and kidney issues can also increase BP other than stress. If any issue among these are found we need to add another medicine along with telmesartan 40mg.

What you are describing is largely expected behaviour for a person on antihypertensive medication. Blood pressure is not a fixed constant â it naturally fluctuates throughout the day based on activity, stress, meals, posture, temperature, and caffeine. No medication can suppress these physiological variations entirely. Readings of 140/80 on a busy day and 150/90 under acute stress are transient and considered acceptable, especially when your morning resting BP is in the normal range (which is actually the most meaningful and reliable measurement). Telmisartan 40 mg is a once-daily medication that works best on baseline resting BP, and stress-induced spikes are largely driven by adrenaline â which is a normal body response the medication does not fully block.
Next Steps
Continue monitoring your BP at home, but ensure correct technique: sit quietly for 5 minutes, no coffee or exercise for 30 minutes before measuring, and take at least 2 readings 1-2 minutes apart and average them. The morning reading before medication is the most important. If your morning readings start creeping above 130/80 consistently, discuss with your doctor whether the dose needs adjustment. Please consult me directly on Practo for a detailed evaluation.
Proper evaluation and personalised advice are needed for your concern. A detailed consultation would be best to provide accurate guidance.

Its considered as normal.because stress and anxiety causes increases heart rate and heart pump the blood fast ,so ultimately bp is increased slightly. So dont take unnecessary stress ,its affect all over body.
