I can provide general advice based on the situation you've described.
For a child with a decayed primary molar, dentists typically recommend extraction if the tooth is severely damaged and cannot be restored. Hereâs a breakdown of your options:
Extraction with Space Maintainer: This prevents the adjacent teeth from shifting and allows the permanent tooth to erupt correctly.
Pulpectomy or Pulpotomy: These are root canal-like treatments for primary teeth, but they are only possible if thereâs enough tooth structure left.
Pain Management: Continue with prescribed medications and avoid self-medicating.
Regular Follow-Ups: Monitor the healing process and the eruption of permanent teeth.
Consult with a pediatric dentist for personalized advice.
Next Steps
Consult a Pediatric Dentist: Seek a second opinion if you're unsure about the current treatment plan. X-ray Review: Ensure a thorough examination to rule out any underlying infection or issues with adjacent teeth. Space Maintainer Placement (if extracted): Prevents neighboring teeth from drifting and allows proper permanent tooth eruption. Monitor Healing: Regular dental check-ups to assess the progress.
Health Tips
Maintain Oral Hygiene: Brush gently and use a soft-bristled toothbrush to avoid irritating the extraction site. Diet Adjustments: Stick to soft foods and avoid sugary snacks that can lead to further decay. Pain Management: Follow the dentistâs prescribed medication and apply cold compresses if thereâs swelling. Educate Your Child: Encourage good brushing habits to prevent future decay.

Hello sir,
I can understand your situation.
But that first primary molar (milk tooth) should be removed there is no tooth structure to save the tooth so as advised by your doctor go for extraction of that tooth followed by space maintainer and the adjacent tooth can be saved by pulpectomy.
Don't give over the counter medicines. Relief will be temporary by medicines.
Thank you for your detailed concern. Based on your description and X-ray findings, the right primary first molar is grossly decayed with only fragments remaining, and the second primary molar is also affected. In such cases, extraction is usually the most appropriate treatment for the decayed tooth, especially when the structure is not restorable and is causing pain.
We understand your concern about pain and your child's sensitivity, but modern dental extractions for children are performed under local anesthesia (or even conscious sedation, if needed), making the procedure painless and safe. Avoiding timely extraction may lead to persistent pain, infection, swelling, or harm to the developing permanent tooth underneath.
Next Steps
Go ahead with the extraction as advised by your pediatric dentist.
If the second molar is also decayed but restorable, a pulpectomy or pulpotomy can be considered depending on the extent of the decay.
A space maintainer may be recommended after extraction, depending on your child’s age and eruption stage of permanent teeth.
Health Tips
Delaying treatment could worsen the infection or affect the alignment of permanent teeth. Children tolerate such procedures well under the care of a pediatric dentist. Please stay assured that your child’s health and comfort will be prioritized.
Hi, the tooth that you have marked is grossly destroyed and no other treatment (like pulpotomy or pulpectomy) besides extraction is possible here. Since you have consulted a Pediatric dentist, managing your lil one on the chair should also not be a problem. You should go ahead with the extraction as keeping an infected tooth in mouth will only cause him pain and affect other teeth as well.
Next Steps
Make sure to get a space maintainer placed after extraction to prevent future problem.
Health Tips
Also the tooth behind the one you have marked also has a huge cavity. The pain could be related to that tooth too.
First of all dont panic..its usual at this age. The tooth is so damaged that it cant undergo pulpal therapy and theres nothing wrong in going for extractiom at this age.
There will be associated infection so taking ibugesic will only relieve pain merely for hrs. So its better to take antibiotics mox 250 for 3 days.
Next Steps
book an appointment for extraction Or
Health Tips
if u dont wish to go for extraction take proper antibiotics as said above and review it . 90 percent cas3s it will resorb on its own.
Hi,
The mark is covering the tooth to view. From what can be seen, the decay is deep and better to follow the advise of the dentist. There are no complications with extractions at a young age. A healthy tooth will erupt. There is no need to worry, get the extraction done as advised. Also there are other teeth that have decay. Get all the teeth treated at the earliest.
Hello
The tooth seems completely decayed and cannot be saved hence extraction is the only option
Don't be worried
Also after extraction get a space maintainer done so that the space for the permanent tooth will be maintained by the appliance
