TSH 7.6 with T4 4.5 may indicate an underactive thyroid, but interpretation depends on the laboratory range and whether you take levothyroxine. With long-standing type 1 diabetes, review the thyroid results and medication rather than changing any dose yourself. Repeat TSH with free T4 as advised and continue monitoring glucose closely.
