Your clinical picture reveals stains and deposits in your teeth ...As the deposits progresses apically it cause gum recession and erosion of bone which cause loosening of teeth...bleeding and swelling in gums are also due to this....you need to go for complete cleaning (scaling) of your teeth....

I would like to educate you and advice you regarding the harmful effects of chewing tobacco.Tobacco chewing is the leading cause of oral submucous fibrosis,leukoplakia and oral cancer. So I advice you to quit the habit.
The stains could be due to the habit.
In addition to stains you have deposits ( calculus) around your teeth which is the reason for bleeding from your gums ,spacing between the teeth and loosening of the teeth. Visit a dentist and get your teeth cleaned ( scaling). After scaling ( cleaning)of the teeth ,the teeth which are very mobile has to be removed(extracted) and replaced with a partial denture either removable or fixed depending on the periodontal condition(bone support ) of the remaining teeth .

go for scaling first ...maintain hygiene ..apply gum astringent at junction of teeth and gums.do mouth wash rinses..
