Hi looks like your aunt it is having a a hemorrhoids which has punctured causing it to bleed heavily. Ideally e c requires a proctoscopic examination in a clinic or hospital by a certified General Surgeon. If there is active bleeding then we put a lignocaine soaked gauze piece to arrest the bleeding ... However If the bleeding is controlled the following  treatment  is prescribed 1) Tablet Daflon 1gram - (three times a day for 4 Days ) followed by( twice a Day for 3 days) followed by (once a day after breakfast for 1 week ) this drug increases the venous tone of the hemorrhoids and reduces bleeding 2) syrup Cremaffin Plus 20ml in the night - this syrup reduces the hardness of the stools which will prevent for the injury to the piles 3) Sitz Bath - take a tub filled with lukewarm water and add half a cup of Betadine solution into it and sit in it for 15 minutes twice a dayA warm sitz bath could be helpful to relieve congestion and edema by aiding venous return from the perianal area. 4) high fiber diet - rich in green leafy vegetables and fruits. Avoid meat and eggs for at least two weeks

depending upon the results of the proctoscopic examination the doctor might advice surgery for just medication and follow up also show the patient requires a blood examination to look for anaemia as the patient has blood a lot through the piles if anaemia is there the patient might also require blood supplements such as iron tablets or if the anaemia is very severe ( Hb<8g%) then blood transfusion is required.

First  priority is to control  heavy  bleeding. She should  take tab trenexa one tab twice daily for 5days  Tab daflon 1000mg once tab thrice daily for 4days and then tab daflon 1000mg  one tab twice daily for 3days  then continue  tab daflon 1000mg  daily. Since  she is  having  heavy  bleeding  she  may need surgery Laser  or stapler  haemorrhoidopexy

she should  immediately  start taking tablets  as suggested  to control  bleeding. High fiber  diet  with plenty of liquids orally  to avoid constipation and straining for stools.

Hi! According to your complaints she is loosing blood during motion that means she will be anemic. So first check her haemogram which is a blood test. Piles needs to be checked by a surgeon and according to the size and grade of the piles treatment differs. So meet a general surgeon. You can give her syp cremaffin 10 ml once daily for few days but this is temporary. She should also take sitz bath which your doctor will explain

But as per my opinion, I need proper history like ,what's her age, duration of problems, any constipation and pain, any associated illness like cardiac, any drug- blood thinner, or any surgery /fever or patches over body.. Actually real treatment always need these answer for better treatment.. I will advise you to see me personally with all available reporta with pt. I need to proper examination first to provide you best treatment.. Don't worry, I have helped lot of patients with this problem.. if somebody have suggested u surgery take a second openion then proceed Because if it's piles , I do a painless lesser or stapler surgery which is mostly become painless and early return to home like office procedures

Ideally if its a diagnosed case of piles, we have to stage the case and treat it. For that, P/R and Proctoscopy should be done. Meanwhile, you should take medicine for constipation like Cremafin plus/ Smuth syrup, along-with tablet Venusmin 300mg three times a day for 10 days. In addition, take cap Daflon 3 times a day and Anovate ointment to apply. Best to get her examined by a qualified surgeon.
