You have to be examined to differentiate whether you have piles or fissure. From history it appear to be fissure, though pile is common in pregnancy and delivery. Treatment of Piles and fissure is Diet, Stool softener syrup, topical ointment and surgery. Diet should be small in portion, salad and green leafy vegetable daily. Syp Duphalac daily for 15 days, first thing in the morning. Initially 30-40 ml then 10 ml daily. Tab Paracetamol in the morning. Ointment Cremagel H or Anometrogyl two or three times and after motion. For initial days seitz bath before going to motion. Stop if you are taking Iron tablet. After this treatment if you does not get relief, consult a surgeon after 15 days.
