I am a 20-year-old college student experiencing cognitive and emotional difficulties for around 7 years. I have severe brain fog on most days, with poor concentration, memory, critical thinking and decision-making. Even small amounts of work can cause cognitive overload, and I quickly become anxious, frustrated and impulsive when I cannot complete something.
I also feel emotionally numb. I rarely experience happiness or sadness strongly anymore, and I struggle to understand or express my emotions. I have become increasingly absent-minded, irritable and unable to tolerate pressure. I often live moment-to-moment and struggle to plan for the future.
I feel as though I have been stuck in “survival mode” for years, but I do not want to self-diagnose. I would like to understand whether this could be related to chronic stress, anxiety, sleep, nutritional/hormonal issues, depression, or another medical/psychological condition. I cannot afford regular counselling, so I am seeking suggestion

Seven years of feeling “stuck in survival mode,” with brain fog, poor concentration and emotional numbness, can be incredibly exhausting.
When even small amounts of work feel like “cognitive overload,” it is understandable that anxiety and frustration quickly take over.
This does not mean you are permanently damaged or incapable of functioning better.

It sounds like prolonged mental and emotional exhaustion. Brain fog, poor concentration, emotional numbness, irritability and feeling overwhelmed can have several possible causes, including chronic stress, anxiety, depression, sleep difficulties, nutritional or hormonal factors, ADHD, or other medical concerns.
Since this has been going on for around 7 years, I would recommend a basic medical checkup along with a psychological assessment rather than trying to self diagnose.
Please don’t see these difficulties as laziness or lack of ability. With the right support, things can improve.

A holistic approach is required, consulting a psychiatrist is a must for physical symptoms and start with relaxation techniques, studying something new, start small but be consistent as much as possible. Incorporate at least stretches for now. Consult also a general physician for full body health check up, I know it is very difficult for you but remember the tunnel is open as you keep going you ll see light, do small positive changes even for 5 minutes is a great way to start your day. Consult all the 3 specialists and venting out about your past tragedies is very important as it will reduce the emotional burden.

Yes, you have been experiencing severe stress, anxiety, and depression but the cause of it is more psychosocial, therefore the treatment will also be more therapeutic and very less medicinal, if any. I believe you're pursuing a degree or a professional qualification which you do not like and have choosen it without much of a thought or realized later that you're not inclined to the field you have choosen. Lot of engineering and medical graduates go thorough a similar phase, trying to adjust to a novel academic, social, and emotional environment that is not only challeging academically but also personally and emotionally. Although, most of them are able to come out of it with little help and struggle, but some of them do collapse or suffer severely. They do need professional guidance and regular therapy to overcome this predicament in their lives. These days almost all educational institutes have a Counselling facility available on campus or some Counsellor visiting the institute 3 days a week. You can consult them and seek help. It is free and most counselors do well when they are trusted and given a sincere chance to demonstrate their skills.
