I have never taken any BP or cholesterol medication before.
I previously have diagnosis with herpes zoster followed by post-herpetic neuralgia and is still taking Gabantin GRS 300 mg and Symbal 20 mg.
Considering my current condition and these ongoing medicines, is it safe to start Telista 80 mg and Razel EZ 10 mg?
Since this is my first BP medication, is starting directly with Telista 80 mg appropriate, or would a lower dose be preferable?

Starting directly with Telista (telmisartan) at an upper-end strength is generally not standard practice for someone who has never taken blood pressure medication before. For a treatment-naive patient, medical guidelines typically advise beginning with an initial, lower strength to assess tolerance and response. While your average blood pressure of 156/86 mmHg indicates hypertension that requires clinical management, initiating therapy at a high strength increases the risk of a sharp drop in blood pressure, leading to symptoms such as dizziness, lightheadedness, or fainting. Gradual titration allows the cardiovascular system to adapt safely.

Consult with a healthcare provider before starting any new medication or altering your treatment plan.

Thank you for giving full details. Telmisartan is a good first choice for you, more so because your urine ACR 46 shows early protein leak. But starting straight at 80 mg is not needed - I would begin with Telista 40 once daily in the morning, recheck BP after 2 weeks and step up to 80 only if readings stay above 140/90. Gabantin and Symbal have no clash with telmisartan. For LDL 174 with your age, a statin is justified; rosuvastatin 10 mg alone usually brings it down enough - the ezetimibe combination (Razel EZ) can be kept for later if target is not reached.
Next Steps
Please do not self start - get one physician visit for the prescription. Keep a home BP diary morning and evening. Repeat creatinine, potassium and lipid profile after 4 weeks of starting. If you get chest pain, severe giddiness or sudden weakness of one side, straight to hospital emergency.
As such it is always desirable to start b.p. medicines in low dose and increase dose after monitoring. Your lipid profile is deranged, adopt life style measures as well.
Please consult a physician physically for clinical examination.
