"Make a gradual exposure to social situations plan for yourself to make reduce my social anxiety" but I've been trying this for a very long time but failed.clearly

Just exposure to social situations will not be sufficient as that is one of the processes.
You may be having other challenges such as low self-esteem or lack of confidence etc. Suggesting to consult a Counsellor for further help.
