Hi, you have type 2 modic changes that’s means it’s chronic condition. if you have only Back Pain then physiotherapy for Core muscles strengthening back extensor + Abdominal + knee + hip muscles strengthening exercises will help to do day to day activities pain free and further worsening of condition And if you have leg pain also then you need to see Spine surgeon, need clinical evaluation. And according to your pain and disability to do daily activities we can suggest you accurate treatment.

1) get evaluated with spine surgeon 2) do Standing Forward and backward bending X-ray will help to access instability in your spine 3) if no leg pain , no leg muscles weakness then physiotherapy and if yes then let spine surgeon take a call - can take 2 -3 Opinion also
