Change life style n diet plan drastically. Tab nexpro RD40 ONE IN EMPTY STOMACH FOR ten days.  Mucain gel10 ml without water nothing by mouth till 30min can be taken 3to 4 times  it's safe. Repet usg after two months.

Do repeat your ultra sound at another set up if you still have pain if its due to gastritis take good diet non spicy and isbagol for good bowel movement Do a hip xray to check for bony pathology around iliac fossa . if there is fever associated with it

More detailed history needed to come to some conclusion and help you with treatment. For fatty liver avoid fatty food, take small frequent meals,  repeat abdominal ultrasound after 2 to 3 months. Get serum lipid profile done

Pain from  2 months is a long time you need further evaluation. It's not because of fatty liver.

Would be because of gastritis. Avoid spicy foods Avoid coffee and tea in empty stomach. Use more of curd and butter milk.
