Consult an orthopedic surgeon, after that physiotherapy to strengthen the muscles is important. Dont do abduction(moving arm from side tooverhead) and external rotation(outward rotation of arm).
Most probably you have instability of shoulder joint due to bankarts or hill Sachs lesions you should get a MRI of your shoulder and consult Orthopedic or arthroscopic surgeon
Your symptoms suggest that some of the soft tissues in the front part of the shoulder were torn during the initial injury and created an instability in the shoulder. So the treatment will involve strengthening the structures in the front part of the shoulder by means of a surgery. The deficit in the shoulder will be well demarcated with the help of an MRI so that should be the next step in the management of your case.
You need an MRI as well as a CT scan for your shoulder considering the long duration of your symptoms . At the same time I advice you to avoid any of the provocative movements that may cause your shoulder to pop out
It looks like a recurrent dislocation (or subluxation). Initial dislocation which had happened 5years ago weakened your shoulder capsule and caused instability. That means your shoulder is unstable. Because of this your shoulder get dislocated often in some angles. Severity of the issue should be ruled by proper investigation like MRI.
Next Steps
You should meet a orthopaedic surgeon with MRI.
Health Tips
Avoid throwing and even bowling too. These will worse the problem.
According to your history you may injury your soft tissue or bone of your shoulder joint. You need to investigation in from of X day and MRl to diagnose where is exact problem. Repeated dislocation is also not good for future outcome .
Looks like you have unstable shoulder. You will need to be investigated for it. First a X-ray of the shoulder will be required to rule out a fracture of the shoulder cup (glenoid), the a MRI of the shoulder will be required. If there is a tear of the cup rim (labrum) then with key hole surgery it can be fixed. But I would like to know how many times have you dislocated your shoulder?
Next Steps
Should visit a arthroscopy/sports injury specialist
Health Tips
Since you are very young a surgery will be required as there is a high chance that without it you will keep dislocating your shoulder.
