Good evening. Is the patient also having difficulty in walking. Is there any tingling numbness in his lower limbs. There is a possibility of compression of the nerve roots or spinal cord which might  be causing pain. If pain is acute, can start with conservative treatment with hot fomentation, analgesic and muscle relaxants.. If pain is severe, should go for further investigation like MRI..
