Shoulder pain with an MRI showing muscle inflammation and excess fluid within the joint suggests an underlying inflammatory process or a soft tissue injury, such as a rotator cuff strain/tendinitis, bursitis, or another shoulder pathology. The exact diagnosis depends on the complete MRI report and a detailed clinical examination. Joint fluid itself is not the disease—it is usually a sign of underlying inflammation.
Next Steps
Please consult an Orthopaedic Surgeon with the complete MRI report for a detailed evaluation. Until then, avoid overhead activities and heavy lifting, continue gentle range-of-motion exercises if tolerated, and follow the prescribed medications. If the pain is severe, persistent, or associated with significant restriction of shoulder movement, further assessment is necessary. You may also book an online consultation with me through Practo for a detailed review of your MRI.
Health Tips
Seek urgent medical attention if you develop fever, marked swelling, redness, severe night pain, or inability to move the shoulder, as these may require immediate evaluation.

Can you share the detailed report here. It looks like inflammatory synovitis. But we need to investigate further for the cause of inflammation. You may need to do some blood tests for it.
Next Steps
Start with some anti inflammatory medication and physiotherpy. Apply ice at home.
It can be because of multiple reasons. Muscle inflammation or tendinosis is seen in small but repetitive muscle injury. Fluid in the joint is due to inflammation of the same. If you have a fever as well, then better consultation in a nearby clinic as it can indicate septic arthritis and should not be ignored. Diabetic and hypothyroidism patients are prone to frozen shoulder which can be the cause of your pain. For now, give hot fomentation, and analgesics.
There is no details regarding type of pain, duration of pain, and other details.
please provide detailed history and please upload the mri film pic and report pic.
