I am not getting proper sleep from last 3-4 years.
It is because of stress anxiety. Due to this I'm unable to think properly. Cloudy thoughts, headache while waking up, problem in making decisions, physical fatigue, forgetting things easily

Most of your symptoms can be because of lack of proper sleep since long period of time. Please try meditation and sleep hygiene. Get your Vitamin D levels checked, if low take vitamin D supplements. If none of this helps please visit a psychiatrist for evaluation of your sleeplessness. Cannot prescribe anything over chat without proper patient evaluation
As per your mentioned complaint seem like you might have depression this will need further evaluation regarding your current situation you should seek help of professional Psychiatrist.
