Question on Meniscus tear
Right knee MRI shows grade ii signal in the body and posterior horn of medial meniscus and Mild thickening of lateral patellar retinaculum with mild lateral tilt of the patella - likely mild features of excessive lateral patellar syndrome. No features of patella alta or trochlear dysplasia. The consulting doctor says there is a tear that has been fragmented. Will it fall under Grade 2 tear and is there any conservative treatment? Pain reoccurs in both knees during squatting, getting up after sitting for long hrs, climbing down stairs, sitting cross legged. Photos of MRI attached.

If the pain is there for both knees , then I could not be meniscal injury . It could be quadriceps pain also. Even though surgical repair is needed for meniscal tear , undergoing initial physiotherapy for few months is not a mistake. Physiotherapy should be only static strengthing exercises and slow movement exercises only. No need of strict bed rest of there is not much of pain .

Never rush for surgery in case of meniscus tear of any grade because most of them heal on their own. Use a hinge knee brace for 6 weeks and avoid full weight bearing on the affected side, use a walking stick or any other walking aid. Take supportive medication and after 6 weeks start gradual physiotherapy.
