Answer by Dr. Jitesh Bhatt MD Dear Patient, At 28 years of age, premature ejaculation (within 5 seconds), difficulty maintaining erection after ejaculation, low libido, and inconsistent morning erections suggest a combined functional + possible hormonal component. 1️⃣ First Understanding Your Condition Premature ejaculation (PE) and low libido can be due to: Performance anxiety / psychological stress Hypersensitivity of glans Low serotonin activity Hormonal imbalance (especially low testosterone) Lifestyle factors (poor sleep, stress, excess masturbation, pornography dependence) Early vascular changes (your borderline high cholesterol is relevant here) Your blood sugar levels are normal, which is good. However, Total Cholesterol 214 mg/dL and LDL 139 mg/dL are borderline high. At your age, this can start affecting penile blood flow over time. --- 2️⃣ Important Tests You Should Do Please get these investigations done: Serum Total Testosterone (morning sample, 8–10 am) Free Testosterone Serum Prolactin LH, FSH Thyroid profile (TSH) Vitamin D3 Vitamin B12 Low morning erections often correlate with low testosterone or poor sleep quality. --- 3️⃣ Lifestyle Correction Plan (Very Important) ✔ 7–8 hours deep sleep ✔ Stop pornography completely ✔ Limit masturbation (not more than 1–2 times/week if needed) ✔ Regular exercise (45 min daily – mix of cardio + strength training) ✔ Weight control ✔ Reduce junk food and sugar ✔ Add nuts, seeds, fruits, leafy vegetables ✔ Omega-3 rich foods Cholesterol improvement alone can improve erection quality significantly. --- 4️⃣ Premature Ejaculation Management Behavioral Techniques: Start–Stop technique Squeeze technique Pelvic floor (Kegel) exercises Practice daily for 8–12 weeks. ⚠️ Do not self-medicate without consultation. --- 6️⃣ When to Be Concerned Seek direct medical consultation if: Erectile difficulty persists >3 months Testosterone is low You have depression symptoms No morning erections at all --- Final Advice Your condition is very common and completely treatable at this age. With proper hormonal evaluation, lifestyle correction, and constitutional treatment, excellent recovery is possible. Do not panic. Early correction gives best results. Stay confident. This phase can be reversed. 💪
