You should consult with your Gynaecologist again. It could be because of some infection in the stitches.
Without a clinical examination it is difficult to find out how extensive the stitches were.
So a physical examination is a must.

Hello ma'am.
Normally, discomfort and itching after childbirth, particularly around the anal area, can result from several issues. While a pinworm infection is one possibility, there are other conditions that can cause similar symptoms, like: Hemorrhoids (common after delivery), Anal fissures (small tears in the skin around the anus), Yeast infections or skin irritation from hormonal changes post-delivery . Persistent pinworm infection (if not completely treated or re-infection occurs)
Next Steps
You can get a general surgeon's opinion to rule out hemorrhoids, if that is ruled out, then you may visit a gynecologist for further examination.
Health Tips
Till then, continue with sitz bath, maintain hygiene, stay hydrated and, if discomfort during bowel movements is a key issue, adding a fiber supplement like psyllium husk (Isabgol) can help soften stools and prevent straining.
Dear Madam, don't worry, it may be due to general infection which occurs post par tum and can be controlled by topical application of ointments. Sometimes the internal stitches they may get inflammed. Pls consult your gynae again for local examination and further treatment.

Based on the symptoms you've described, it seems that the issue you're experiencing could be related to a persistent infection or irritation. Pinworm infections typically cause itching, particularly around the anal area, which aligns with what you've mentioned. However, since the symptoms reoccur after discontinuing the medication, it could also be related to incomplete treatment, an incorrect diagnosis, or another condition such as hemorrhoids or an anal fissure.
I recommend that you revisit a specialist, such as a gastroenterologist or a proctologist, who can perform a thorough examination. This would help in determining if additional treatment is needed or if further testing is required to get to the root cause of your discomfort.
