Yes, it is possible not to have a fistula even after two perianal abscess episodes. Since both were small, resolved quickly with antibiotics, did not require drainage, and you have been symptom-free for 10 months, the likelihood of an underlying fistula is low at present. Many fistulas develop after inadequately drained or persistent abscesses, which does not seem to be the case here.
Next Steps
• No treatment needed right now if asymptomatic
• Observe for any recurrence
• Consult a surgeon only if symptoms reappear (pain, swelling, discharge)
Health Tips
• Watch for warning signs: recurrent pain, pus discharge, non-healing swelling
• Maintain good perianal hygiene
• Avoid constipation and straining
• Seek early evaluation if any new symptoms develop
Fistula usually presents with persistent or recurrent symptoms such as pain, swelling, discharge of pus, or repeated abscess formation. Since you have none of these symptoms for a prolonged period, there is no current evidence to suggest a fistula.

Consult a colorectal surgeon if any new symptoms such as pain, swelling, or discharge develop in the future
