It looks like unhealed sprain of patellar tendon which connects your patelle bone and tibia …. If you can get an xray we can ruled out any unhealed avulsions of the tendon… If the pain is unbearable get a consultation immediately If the pain is mild start with strengthening excercises the pain will gradually decrease but you have to rest the knee for proper healing.. If you cannot take rest use a knee cap or knee brace
Next Steps
Rest with Knee braces and strengthening exercises

Based on your history, pain persisting for 4 years after a fall is unlikely to improve with rest or ice alone. Pain localized over the patellar tendon (below the kneecap) can be due to chronic patellar tendinopathy, Hoffa’s fat pad impingement, patellofemoral cartilage injury, an old partial patellar tendon tear, or other intra-articular pathology. A proper diagnosis is essential before deciding on treatment.
Next Steps
I recommend consulting an Orthopaedic Surgeon (preferably a knee specialist) for a detailed clinical examination. If not already done, an MRI of the knee is advisable to identify the exact cause of your persistent pain. Based on the findings, treatment may include structured physiotherapy, activity modification, medications, PRP injections (in selected cases), or rarely arthroscopic surgery if there is associated intra-articular pathology. If you have your previous X-rays/MRI reports or would like a detailed assessment, you may book an online consultation with me through Practo. I can review your reports and guide you regarding the most appropriate treatment plan.
Health Tips
Until the diagnosis is confirmed, avoid repeated squatting, jumping, running, and excessive stair climbing. Persistent knee pain for several years should not be managed with painkillers alone, as identifying the exact cause is the key to successful treatment. This wording is professional, helpful, and naturally encourages the patient to consult you without being overly promotional.

It might be partial tear which has not healed properly. Also may have been a bony injury which was ignored. Get xrays done and checked by orthopedic surgeon
