15years is a long time indeed. Have you been taking these medicines in proper doses ? Have you tried CBT or any other form of therapy with medicines ? The medicines you've mentioned here are good medicines. But if taken in sub-optimal doses, they'll not help.
Next Steps
Do consult a psychiatrist and also consider making some lifestyle changes - exercise regularly, try some relaxation exercises, yoga etc. cut down on Alcohol/smoking if you do

Hi Sir Thanks for seeking help here. After careful evaluation of your history, I would like to  convey about something, none of this medication is 100 percentage efficacious in panic disorder. Medication are not the solution to panic disorder. The maximum effectiveness with medication is 60 percentage. Other behaviour strategies n cognitive therapy usually helps in getting more better results. I would like suggest you sir, first exclude all organic causes, then we may focus on psychological issues. Happy to help you... Regards Dr. Jitendriya Practo

Firstly it’s good you’re taking your treatment. Consult a psychiatrist for treatment decisions rather than making them on your own. Together with counselling and medicines it’ll be better way to deal with your issues.
